USA Hockey consent to treat form USA Hockey waiver of liability Initiation Level Skaters Contact: Paul Stehr (651) 982-9229 home threepsana@msn.com (home email) What is initiation hockey: FLHA’s initiation program is designed to provide a fun and positive hockey experience for our youngest players. Our program teaches hockey skills at each level. The first ice time will be Saturday, October 6th, 2007. Initially, skaters will be assigned times according to their hockey age (their age as of June 30, 2007). Level 1 – 4 and 5 year old’s program teaches the most basic skills of hockey and is intended for skaters with little or no skating experience Level 2 – 6 year old’s program has skaters with prior skating experience and will be conducted in a team format. Focus will be continued individual skill and team concepts with skaters able to skate forward at a reasonable pace, stop in one direction, pass, positioning and ½ ice games included Level 3 – 7 year old’s program is for skaters with more advanced skating skills and will be in a team format. Still focus on individual skills development. Team concepts should include: defensive zone break outs, offsides, passing and positioning,etc. skaters should be able to skate forward and backwards attempting to do crossovers and stopping in both directions Level 3 – 8 year old’s program is designed for skaters who have more advanced skating skills and solid grasp on team concepts. Most players will be in their last year of initiation with some exceptions of level 3 players able to move to fill teams with like skilled players. Ice times will still focus on individual skill development, with the addition of more on team concepts. Team concepts should include positions, territorial defense, fore-checking, back checking, and basic defensive coverage. Skaters should be able to skate forward and backward, forward crossovers while handling a puck, backward crossovers, backward stops and stopping in both directions. All skaters shall wear full equipment at all times on the ice. Full equipment includes: Helmet with cage face guard Shoulder pads Hockey gloves Breezers Shin/knee pads Mouth guard attached to cage Elbow pads Athletic supporter w/cup Hockey skates Hockey stick Large equipment bag Neck guard (optional) Jerseys and socks provided by FLHA During the October skill sessions, there will be one mentor coach with 4 or more coaches on the ice running skill drills. Some skaters will be moved so that they are skating with like skill levels. As skaters in October skill sessions advance, they will be moved to the next appropriate level to continue their skating education. Parents should be prepared to adjust their child’s schedule, as indicated by the mentor coaches, during this time. At the end of the skill sessions, each skater will be evaluated by the HEP evaluation sheet on the last weekend of October, with the exception of the level 1 skaters, then placed on teams by November and contact by his/her coach. October ice sessions will consist of skills practice, as it did during the 2006-2007 season. There will be the traditional In-House Mite Jamboree in December (tentative dates are the 15th and 16th). During the months of January and February, we will have a ratio of three practices to one scrimmage/in-house games for the Level 1 and 2 and 3. The Level 4 teams will be allowed to play 7 to 12 games against District 2 opponents. These games will be timed, scored and refereed (if agreed upon by coaches).* The season will end for all teams with another Jamboree, tentatively, February 23th and 24th, 2008. All initiation teams must have a team manger to manage pictures, fundraising and jerseys, silent auction item, etc. All coaches (and parents) on the ice must have a Level 1 coaching certification by Dec. 1, 2007. Please visit www.USAHockey.com for dates and times of future coaches certification clinics. Forest Lake Hockey Association will reimburse the cost of the clinic fee. All coaches must wear HELMETS on the ice. *Note: For Level 4 teams there will be a mandatory coaches meeting with District 2, for the purpose of explaining the goals and guidelines and game scheduling. There will be a fee associated with the games where referees are used. Parent volunteers will be utilized to run the clock and keep the scorebook during the games. Most levels will be skating twice a week with the exception of the level 3 and 4 teams may be charge for additional ice times. Outdoor ice should be used when available for it is free. Team pictures will be Nov. 19th and 20th, 2007 Please note there there will be additional fundraisers throughout the season. The bottled water fundraiser is payable at registration. In addition, each family will be responsible for paying for at least $120.00 in raffle tickets per child. These tickets may be sold, but in the event tickets remain unsold the family must pay the difference. The actual amount of the raffle ticket fundraiser has not been determined. Minnesota Hockey and Minnesota District Age Charts for 2007-08 Season Players, to be eligible, must have been born on or after the stated date.
